This is a mandatory form that must be completed by students who have met the requirements for graduation at Highland Community College. It is important to complete the application on time to ensure eligibility for graduation and to have the diploma awarded. For questions, please reach out to the Office of the Registrar.
Are you a student who has been diagnosed with a permanent and/or temporary physical, medical, psychological, or learning disability, and are seeking accommodations?
It is the intention of Highland Community College to work toward full compliance with the Americans with Disabilities Act and to make facilities and instructional programs accessible to all people, and to provide reasonable accommodations according to the law.
Students should understand that it is their responsibility to self-identify their need for accommodation(s) and that they must provide a current, comprehensive diagnosis of a specific disability or medical condition from a qualified professional in order to receive services. This documentation must be submitted prior to services being authorized.
Pursuant to provisions of the Federal Family Education Rights and Privacy Act of 1974 (Public Law 93-380), you may grant permission for release of personal data information and information concerning your educational program, to anyone other than yourself.
This appeal procedure is available only for the review of allegedly capricious grading.
For students who have completed at least 40 credit hours. This verifies that the student has met or will meet the requirements for their degree.
Students should complete this form before enrolling with their advisor for their final semester to ensure they are on track for graduation. Technical programs do not require a graduation check.
For student who are making a passing grade and cannot complete the required work in a course because of illness or some other approved reason
To request an administrative withdrawal during the current term when a medical condition or other hardship makes it impossible to continue.

Please submit the following form at least one week prior to the start of the semester for which you are requesting the overload.
Students suspended from receiving Federal Financial Aid for not meeting Satisfactory Academic Progress (SAP) may appeal to regain Federal Financial Aid eligibility for one semester. The deadline to submit a SAP Appeal (and Academic Plan, if applicable) is the first day of the student’s classes in the semester for which the student wishes to receive financial aid. Any Appeals received after that date will be considered as submitted for the next semester.
This form should be submitted if you have lost an academic scholarship and would like to appeal due to unforeseen circumstances which caused you to drop below minimum scholarship requirements.
*Student athletes with a cumulative GPA below 2.8 are not allowed an appeal.
Tuberculosis (TB) is a bacterial infection that attacks the lungs and sometimes other parts of the body. An individual with active tuberculosis can spread the disease by coughing, sneezing, speaking, laughing and singing; distributing the bacteria in the air to be inhaled by individuals in close proximity.
